Application by a person aged 18 years and upwards who wishes to become an Irish Citizen by the registration process laid down in Section 27 of the Irish Nationality and Citizenship Act, 1956, as amended.
Please read the attached checklist carefully. The checklist outlines the documents required to assess eligibility for Foreign Births Registration.
This section must be signed in the presence of the witness completing Section E.
I declare that the particulars in this application are correct and that I wish to be registered under Section 27 of the Irish Nationality and Citizenship Act.
This must be completed by a professional who is personally known to the applicant.
Note: Certificates in a language other than Irish or English must be accompanied by a certified translation.
Witness must be from approved list (doctor, teacher, etc.) and not a relative. Must sign and stamp two photos.
Write your application number on the envelope clearly.
